Important

This is a single-session form. Once you begin the application, you won’t be able to save your progress and return to it later. So please ensure you have all the documents you need before you start:

  • Mandatory documents:
    • One (1) cover letter – A cover letter written by you, the nominator, is required. It should explain how the candidate fits the stated criteria and should be no more than 2 pages.
    • Two (2) letters of support – These 2 letters of support, written by supporters other than the nominator, should reinforce the nomination of the candidate. Each letter should be no more than 2 pages.
  • Optional: Additional supporting documents (e.g., candidate’s curriculum vitae of no more than 8 pages, candidate’s resumé, links to news articles, etc.), if available, may be submitted.
